  1. License on Form 20 is issued for the sale of Allopathic drugs by retail other than those specified in Schedule C, C(1) and X. (Form-19)
  2. License on Form 20-A is issued for the sale of restricted Allopathic drugs by retail other than those specified in schedule C, C(1) and X. (Form-19A)
  3. License on form 20-C is issued for sale of Homoeopathic medicines by retail. (Form-19B)
  4. License on Form 21 is issued for retail sale of Allopathic drugs specified in Sch C & C(1).(Form- 19)
  5. License on Form 20-F is issued for retail sale of drugs specified in Sch. 'X'. (Form-19-C).

Complete Checklist of RETAIL/PHARMACY DRUG LICENSE

  1. Copy of Adhaar card/voter identity
  2. Pan card of company /director firm & partners/proprietor
  3. Property papers copy/Ownership Proof (if owned property)
  4. Rent agreement copy (if rented property)
  5. Electricity Bill/ water bill /telephone bill
  6. MOA/AOA ( in case of temporary copy of partnership in case of partnership) or GST/ST2 in case Proprietorship Firm.
  7. 2 Photographs white each (White background) Directors or owner
  8. List of items in which the firm or organization in which firm will deal.
  9. Qualification Certificates of Pharmacist B/D Pharma
  10. Appointment Letter In case Pharmacist is an employee on their Letterhead.
  11. NOC from all concerned bodies.
  12. Passport Size Photo of Competent Person
  13. Directors ID Proof , Pan Card and Passport Photo
  14. Directors Qualification
  15. Board Resolution for authorized signatory
  16. MOU/ AOA
  17. List of Directors
  18. NOC Affidavit from Directors
  19. Application letter for applying wholesale drug license
  20. MCD Conversion charges Receipt/ Commercial property tax receipt.
  21. Shop & Establishment
  22. MPD affidavit

Minimum carpet area

Retail Drug-License - 10 square meter / 110 Sq. Ft.

Storage Facilities

Adequate storage facilities including cupboards with glass doors, racks refrigerator is required
