The first step is to get the provisional registration for the clinical establishment. Afterwards, when the clinical establishment confirms all the minimum standards as prescribed by the government, it can apply for permanent registration. The documents required for the provisional registration are as follows-

2. Identity Proof of the Owner
3. Address Proof of the Establishment
4. Registration Certificate of the Person-In-Charge
5. Qualification Certificate of the Person-In-Charge
6. Proof of Payment of the Registration Fee
