• Date of Birth Proof of Applicant (Any One is mandatory)-
1. Passport
2. Driving License
3. CMO / Doctor Report
4. Aadhaar Card (Verified DoB)
5. Nursing home/Hospital Report/Vaccination card
6. SSC from recognized board by GoI
7. Certificate from School signed by Principal on School Letter Head


• One Passport size Coloured Photograph of the applicant (Size 5cm x 4.5cm Or 2” x 1.75”)
1. Should include full face, front view and open eyes
2. should be in a plain white or off-white background
3. should have a natural expression (closed mouth)
4. should be of full head from top of hair to shoulder
5. shouldn’t be shadows on the face or background
6. should not include sunglasses or hats
